Certain recommendations made to Chelmsford Prison after the death of inmates haven't been acted on, 麻豆社 Essex has learned.
There have been 12 suicides there in the last five years, the third-highest total in England and Wales. The Ministry of Justice says it has invested more than 拢1bn to try and improve the systems in prisons and train staff better.
Ben Cathorne took his own life while he was detained at Chelmsford last October. His brother Mark says he wants answers.
