Girl, 4, died after hospital concerns - inquest

A four-year-old girl with a rare genetic condition died six months after her mother reported concerns about her treatment, an inquest has heard.
Summer Rae Mant, from Merthyr Tydfil, had and died on 21 September from multi-organ failure and a brain injury.
Coroner Graeme Hughes heard that Summer Rae had been admitted to Prince Charles Hospital in March 2024, where her mother reported concerns about her treatment.
An internal investigation is under way by Cwm Taf Morgannwg health board into the care she received.
The inquest was told that Summer Rae had been transferred to Bristol Children's Hospital where she received further treatment before having end-of-life care at T欧 Hafan children's hospice in Sully, Vale of Glamorgan.
Mr Hughes said he had a duty to investigate where a death may have been contributed to by "treatment or what the law determines culpable human failure".
The inquest was adjourned until a future date.
